Psychological Evaluation for Patients Struggling with Infertility
Because of the psychologically demanding nature of infertility treatment, it has been recommended that psychological services be made available to patients struggling with infertility, egg/sperm/embryo donors, surrogate mother, etc.
Gestational Carrier (GC) Psychological Evaluation:
The evaluation of a potential GC includes the administration of personality testing and a 2-3-hour structured clinical interview. The psychological interview includes questions outlined by the American Society for Reproductive Medicine (ASRM). If the GC has a spouse/partner, that individual also participates in part of the clinical interview. Your process is determined by the referring clinic or agency. This can be completed in person or by telehealth.
Intended Parent (IP) Counseling Session:
According to the American Society of Reproductive Medicine (ASRM), intended parents should attend counseling prior to an egg or sperm donation procedure. This is an opportunity for IPs to receive education, and discuss concerns so that they can comfortably and confidently approach their path to parenthood. This is a 60-90 minute counseling session that can be completed in person or by telehealth.
Egg Donor Psychological Evaluation:
The American Society for Reproductive Medicine (ASRM) recommends that all prospective donors undergo a psychological evaluation during screening to limit the possibility of emotional harm. Psychological screening is generally aimed at identifying emotional problems, evaluating donor motivations, and verifying that women have a thorough understanding of the physical, psychological, and legal risks that could result from donation. The psychological evaluation includes the administration of psychological testing and an in-person or telehealth interview with the egg donor and their spouse or partner.
Egg Donor Recipient Psychological Evaluation:
This evaluation aims to help the intended parents discuss how they feel about having a child that is not 100% genetically related to them, and evaluate the level of their commitment and readiness psychologically. The psychological evaluation includes the administration of psychological testing and an in-person or telehealth interview with the egg donor receipt and their spouse or partner.
Process of the Evaluation
The evaluation begins by obtaining personal information, such as marital status, occupation, education, and interests. The screener will also find out about their self-care routines, such as diet and exercise. Patients will also discuss their stress levels, the current causes of stress, and the demands on their time.
Both medical and psychological reviews of infertility experiences will be discussed. Patients’ opinions about egg and embryo donations will also be assessed. The patient’s background and family history are then collected, such as:
Past or present mental health issues with self and family members
Issues now or in the past with alcohol and/or drug use
Legal problems
Any kind of abuse that the patient has experienced
A family history of conflict or significant crises
Other Information
At least one psychological assessment will then be administered in person or through telehealth sessions. The client is required to answer 500+ multiple-choice questions, and it usually takes 90-120 minutes to complete. In most cases the process can be completed in the allotted time period, however, sometimes additional counseling or psychological testing may be required.
After the evaluation, a full report with the signature of a licensed psychologist will be provided.

Will my insurance cover the testing?
Many insurance plans cover psychological testing. Some require a pre-authorization, which is something you should contact your insurance to request before the appointment for an assessment. If they don't cover it, then you will need to pay out of pocket.
